Top 5 Cimetidine Drug Interactions

Cimetidine was the first widely used H2 receptor antagonist and remains available over the counter for heartburn and acid reflux. While it is effective at reducing gastric acid secretion, cimetidine has largely fallen out of favor because it inhibits multiple cytochrome P450 enzymes, including CYP1A2, CYP2C19, CYP2D6, and CYP3A4. This enzyme inhibition can significantly increase serum concentrations of many medications and raise the risk of adverse effects. While this is a drop in the bucket of the number of drug interactions associated with cimetidine, here are five clinically important cimetidine drug interactions.

1. Warfarin

One of the most important cimetidine interactions involves warfarin. By inhibiting hepatic metabolism, cimetidine can increase warfarin concentrations, resulting in elevated INR values and a substantially greater risk of bleeding. Patients may present with bruising, epistaxis, gastrointestinal bleeding, or more serious hemorrhagic complications. Whenever cimetidine is initiated or discontinued in a patient taking warfarin, INR should be monitored closely, and warfarin dose adjustments may be required. Whenever possible, consider using famotidine instead of cimetidine to minimize this interaction.

2. Phenytoin

Like warfarin, phenytoin has a narrow therapeutic index, making this interaction especially concerning. Cimetidine slows phenytoin metabolism, leading to elevated serum concentrations and an increased likelihood of toxicity. Early signs include nystagmus, ataxia, dizziness, slurred speech, and diplopia. More severe toxicity can progress to confusion or altered mental status. Monitoring phenytoin serum concentrations and assessing for neurologic adverse effects is recommended when these medications must be used together.

3. Tizanidine

Tizanidine is a medication metabolized through CYP1A2 and can accumulate substantially when administered with cimetidine. Elevated tizanidine concentrations may cause excessive sedation, dizziness, profound hypotension, and bradycardia. Older adults are particularly susceptible to these adverse effects and may experience falls or syncope. If acid suppression is necessary, switching to famotidine is often a safer alternative than continuing cimetidine.

4. Metformin

Metformin is commonly used in the management of diabetes. Cimetidine administration can decrease metformin renal tubular clearance and ultimately cause metformin concentrations to rise. If this happens, the patient becomes more at risk for lactic acidosis from accumulation of metformin. This interaction is more concerning if patients already have underlying renal dysfunction.

5. Citalopram

Citalopram concentrations can rise when cimetidine is added to a patient’s drug regimen. CYP2C19 is inhibited by cimetidine (excellent board exam nugget), and this is a critical enzyme in the breakdown of citalopram. One of the major concerns with elevated citalopram concentrations is QTc prolongation. Because of this drug interaction, it is recommended to use a maximum dose of 20 mg for citalopram.

Bottom Line

Although cimetidine remains an effective treatment for heartburn and gastroesophageal reflux disease, its extensive inhibition of cytochrome P450 enzymes makes it one of the most interaction-prone medications commonly available without a prescription. These 5 interactions are the tip of the iceberg, and healthcare professionals should carefully review medication profiles before recommending cimetidine, particularly in patients taking anticoagulants, anticonvulsants, cardiovascular medications, or drugs with narrow therapeutic indices. In many situations, famotidine provides similar acid suppression with far fewer clinically significant drug interactions, making it the preferred H2 receptor antagonist for most patients.
