Morphine Concentrations – Life-Threatening Errors Can Happen

Morphine is one of those liquid medications where concentration matters. A lot. While many medications come in one standard concentration, the problem is that morphine comes in multiple concentrations, and they can look surprisingly similar when you are moving quickly through a medication cart, MAR, pharmacy order, or hospice kit. Throughout my career, I’ve seen mistakes with morphine concentrations, and I just saw another one recently.

Morphine 10 mg/5 mL contains 2 mg/mL. That is very different from concentrated morphine, which is 20 mg/mL.

If someone is expecting morphine 10 mg/5 mL and accidentally administers 1 mL of morphine 20 mg/mL, they have given 20 mg instead of 2 mg. That is a 10-fold difference! There is a huge opportunity for overdose mistakes.

Think about a resident with an order for morphine 5 mg. With the 10 mg/5 mL product, the volume would be 2.5 mL. With 20 mg/mL concentrated morphine, 5 mg is only 0.25 mL. Those volumes are dramatically different. This becomes particularly concerning in long-term care, assisted living, hospice, and other settings where concentrated morphine may be used for end-of-life symptom management.

One of the biggest safety problems is relying on the volume alone. “Give 2.5 mL of morphine” is not a complete safety check unless you also confirm the concentration. The medication should be verified by strength and concentration, not just the drug name and volume.

When liquid morphine is involved, always pause and ask: What is the concentration? What is the ordered dose in mg? And does the volume I am about to administer actually match those two things?
