Medication Reconciliation – Why so many changes?

Medication reconciliation is a major problem! I outline some things to look for in this short video! – Enjoy

  1. Lily

Hello from Finland! I work in a community pharmacy and Im interested in clinical pharmacy. Really enjoy your blog. Keep up the important work!

Thanks Lily! It is incredibly rewarding and enjoyable 🙂

  2. JiEun

Thank you so much for your video. It was informative as usual. My first thought on fluoxetine was another GI bleed risk from SSRI. But despite that, starting on antidepressant without burning indication should always raise the question.

  3. gaber

thank you Eric for the video. My thought is , it also takes time for fluoxetine(proxy ) to fully work, and if you stop clopidogrel (Plavix) makes sens but for how long because we do not know what was the indication for Plavix. As you explain there would be some questions depending on your site of work, and of course information is critical .

  4. Sean

    You’re the man, Eric!

    • Eric Christianson

thanks Sean! good to hear from you!
