Happy Veteran’s day! Narcweek begins now, thanks for all the support! I’m still seeing problems and questions on Duragesic (fentanyl) patches out there, so hopefully I can bring some examples over the next few nights that really emphasize some of the mistakes I see out there in the real world. The first thing I must highlight is how potent fentanyl patches are! A 25 mcg/h patch has a ballpark equivalent to oral morphine of 60-134 mg daily per Lexicomp! A 50 mcg/h is approximately double that in oral equivalent of morphine per day! A 100 mcg/h patch oral morphine equivalent is in the ballpark of 315-404 mg per day! So if a patch is lost whether you are at home, an assisted living, nursing home, or other institution, you better take it seriously!
